New Patients
Gilbert Physical Medicine welcomes new patients in Gilbert, AZ and the surrounding area. You will know you are in good hands from your first visit with our care team. During the consultation, we will get to know you and your health issues before we match you with the medical care necessary to overcome your chronic pain, restoring health and energy. As a new patient, we ask you to fill out the new patient information forms on this site. If you need any help with the forms or have any other questions, call us at 490-545-0000. To schedule an appointment, simply call us during business hours and our friendly staff will set you up with a time that is convenient for you. Our staff can be reached at 490-545-0000 to help you in scheduling the consultation. If you have any questions, we are happy to help you with those when making your appointment. If it is after business hours, feel free to contact us by email or call and leave a message. Someone will get back to you at the earliest possible convenience.
What to Expect
When you arrive at our Gilbert, AZ office, a team member will warmly greet you and check you in. If you have already completed the new patient paperwork, please have a seat in one of our comfortable waiting room chairs, and we will call you in when it is time. However, if you have questions or need help filling out the form, do not hesitate to ask. Our team members will be happy to assist. Remember to have your photo ID and insurance information with you. For all future visits, you will simply need to check in when you arrive.
